The Hospitalist Case Management Coordinator works independantly to plan, implement, and/or coordinate quality patient care and cost effective utilization of healthcare services. The Hospitalist Case Management Coordinator will have principal responsibility for the following key functions: facilitate communication between key internal and external personell; coordinate patient care needs during hospital stay; assist in developing and facilitating plan of care post discharge; and facilitate patient transition to existing community resources post discharge. The Hospitalist Case Management Coordinator applies nationally accepted criteria that include general rules and time definitions to determine appropriate levels of care and to illuminate resource utilization practices within provider organizations. Provides supervison of clerical support. Minimum of (3) three years current experience in acute and/or Managed Care. Utilization Management/Discharge Planning/Clinical Case Management experience preferred. Education level necessary to obtain required licensure and certifications. Bachelors Degreee preferred. Education level necessary to obtain required licensure and certifications. National Certification or actively pursuing a credential specific to area of practice is preferred. Certified Professional in Healthcare Quality (CPHQ), Certified Case Management (CCM), or Certified Professioal in Utilization Review (CPUR) preferred. Skill and proficiency in applying highly technical and critical thinking principles, concepts, and techniques that are central to the Nursing profession. Cognitive and psychomotor capabilities in the diagnosis, treatment, and evaluation of patients and families. Advanced communication and interpersonal skills with all levels of internal and external customers, including negotiation and conflict management skills. Interpersonal skills to obtain information appropriate to asses patient/family age, needs, and range of patient treatment and care. Computer and basic statistical skills. CA RN and BLS. LVN is the minimum licensure requirement for incumbents prior to 7/1/03.
