Clinical Director for Adult Transitional Care Program Job Listing at BAYADA Home Health Care in Burlington, NJ

BAYADA Home Health Care is seeking an energetic, self-motivated Registered Nurse to lead our Transitional Care Program for our Adult Home Care specialty practice nationwide. This innovative Liaison program provides clinical management to ensure the safe transition of catastrophically ill patients from rehabilitation facilities to the comfort of their homes. This clinical leader will incorporate a team based approach to establish BAYADA as the provider of choice for health care facilities, insurers, referral sources, and the community for catastrophically ill clients in need of home care services.

Responsibilities

• Demonstrate and communicate the core values of BAYADA Home Health Care and The BAYADA Way.
• Ongoing development, rollout, training and evaluation of Adult Transitional Care Program nationwide.
• Research clinical and health care trends, competition, clinical processes and products to drive performance improvement of program.
• Utilize key metrics to evaluate and ensure success of the overall program.
• Provide clinical leadership to transitional care nurses and field nurses involved in providing services to high tech catastrophically ill and disabled clients.
• Collaborate with Division Directors and Directors to hire, orient and mentor Transitional Care Managers for BAYADA Home Care service offices.
• Participate in performance appraisals and guide staff through the Talent Development Program process.
• Identify educational needs and develop ongoing training to develop Transitional Care Managers and direct care nurses.
• Collaborate with marketing team to provide clinical expertise to enhance overall marketing strategy for high tech catastrophically ill adult clients.
• Build and maintain relationships with key referral sources
• Collaborate with Liaisons and Transitional Care Nurses of other BAYADA specialty practices including Home Health and Pediatrics to ensure proper coordination of services

Qualifications

• Must have a current RN license, BSN preferred
• Five (5) plus years of clinical experience as a registered nurse
• Two (2) years of clinical liaison experience in a rehab setting.
• Discharge planning and/or case management experience is required
• Previouos Home Care experience preferred
• Demonstrated clinical leadership experience
• Must be clinically competent in assessment, planning, delivering and evaluating individualized care for clients in any state of health or illness.
• Must possess case management skills to help clients achieve optimal well being at home, as well as identify current and potential complications to prevent rehospitalization.
• This position will be based in NJ, but qualified candidates must open to travel to facilities and offices as needed
