BAYADA HOME Health care is currently seeking an experienced home health care Registered Nurse, RN for a Per Diem position as a home care Nurse case Manager providing home visits for our office in Providence Rhode Island. Our Providence Office seeks skilled registered nurses to cover our north territory. This consist of northern Kent and Providence counties. Prior home health care experience strongly preferred. As a home health care nurse, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to patients, affording them the opportunity to receive the medical care required to remain at home.

• Making home visits to clients in designated Northern Kent and Providence counties
• Performing assigned duties, including administration of medication, would care, treatments and procedures
• Monitoring clients conditions; reporting changes to Clinical and Client Service Managers
• Follow up with, executing and properly documenting doctors' orders
• Perfomring client assessments as necessary.
• Strong candidate will have OASIS experience

• A current Rhode Island RN license. A minimum of one year of recent , verifiable clinical (medical/surgical) experience
• A graduate of an accredited and approved nursing program as indicated by school transcript or diploma
• Deomonstrated abliity to read, write and effectively communicate in English
• Ability to work independently and manage time effectively
• Excellent organizational, interpersonal and communication skills
• Solid computer skills; prior expereince with electronic medical records (EMR) preferred
• Ability to travel to casses assigned with the territory
