Qualifications General Work Experience: Minimum of 3 years experience in an Inpatient pediatric setting is preferred. Knowledge of case management, utilization management and assessment tools is a preferred plus. Excellent leadership and interpersonal problem solving skills. Ability to work with a wide variety of clinical personnel required. Strong knowledge of admission processes is preferred. Excellent verbal, written, and phone skills. Ability to meet deadlines and work under pressure. Knowledge of personal computer, including word processing, spreadsheet applications, and other communication software (e-mail). The ability to learn IT systems including the Electronic Medical Record, Electronic Bedboard, and Patient Progression report system. Weight Lifted/Force Exerted: Up to 25 pounds: Up to 1/3 of time Job Overview The Transfer Center Nurse (TC RN) provides the assessment, planning, facilitation, and advocacy for optimal clinical and financial outcomes in the transfer and placement of patients to Children’s Hospital Colorado. The TC RN collaborates with the patient, family, other healthcare team members, community providers, and community resources and payors, resulting in an appropriate level of care, placement, and reimbursement. The TC RN assists with the appropriate use of resources both within the facility and external to the facility. Communication and collaboration may include coordinating admissions and transfers with referring facilities and providers, InterQual assessment, clinical assessment, transportation and insurance verification. Electronic Clinical Documentation is utilized to document and communicate the admission to Patient Access, our Physicians, and RNs. The TC RN may also be involved with other services including, but not limited to collaboration with Managed Care, Insurance Verification, Health Information Management; data aggregation and analysis related to length of stay, outlier days, avoidable days, care delays and denials overturned or upheld; education and case management orientation for RNs, residents, physicians, students, and other departments.
