Title: Social Worker - MSW-Home Health
Location: TX-Lufkin (Houston region)
Provision, direction, supervision, and evaluation of medical social services to clients in the home in accordance with agency policies and procedures, applicable health care standards and governmental laws and regulations. Instructs, treats, observes, and evaluates clients exhibiting significant social and emotional problems affecting their health status. Instrumental in assisting home care patients to find ways of coping with the socioeconomic problems so as to maintain or improve their physical wellness.

Education:
Master's Degree in Social Work required. Licensed in Clinical Social Work (LCSW) preferred.

Experience:
Two year's clinical experience.

Licenses/Certificates: Current CPR card, current driver's license, current automobile insurance.
