Job Type: Full Time Position Summary:
Functions autonomously and in collaboration with all members of the healthcare team to coordinate quality, cost-effective care and minimize fragmentation of the healthcare delivery system. The goals of Care Management are to optimize the patient’s level of wellness and independence, enhance quality of life, prevent unnecessary hospitalizations or service duplication, provide continuity of care, promote cost containment, enhance quality, and improve clinical and financial outcomes primarily through telephonic interactions. The Health Manager is the link between the member, the providers, the payer, and the community. All members will be assigned a Health Manager upon engagement. Required Qualifications:
Minimum of 5 years clinical experience. Preferred Qualifications:
* Unrestricted license as registered nurses in the state where they are employed. Current Certified Case Manager is a plus. * Working knowledge of funding, resources, services and clinical standards relative to specialty. *Ability to evaluate program delivery and make recommendations for enhancements. Education:
Bachelors or Associates degree in Nursing. CCM preferred. Business Overview:
