Responsible to the Clinical Services Director/Clinical Services Manager to provide patients and families psychosocial care regarding economic, social, and emotional needs. Implements and coordinates all social service activities relating to the social aspect of the program.

Education

For hospice services employee must have Masters of Social Work from accredited school.

Registered as Social Worker in the State (according to State Guidelines).
Demonstrated ability to work well with an interdisciplinary team.
Excellent communication skills, both verbal and written, interpersonal relations, and organization skills.

At least one year of experience in varied multidisciplinary settings in the health care field. Experience working with patients and families coping with terminal illness and death.

Lifting 50 pounds maximum with frequent lifting and/or carrying of objects weighing up to 25 pounds; Walking, standing, talking, hearing, pushing, reaching, seeing stooping, smelling, depth perception, color vision.

Necessary on a daily basis.

Exposure to disagreeable conditions including odors, infection, illness, and physical contact from disruptive patients.
