Position Summary
Under the direction and supervision of the Home Care Manager and Lead Home Care Manager, the LPN administers medications and treatments, draws blood, provides participant and family education and supports those activities necessary to the preservation of organization within the home environment.

Essential Functions and Work Duties
The following essential functions of this position are performed personally, in cooperation with your supervisor, and/or in coordination with other staff. Additional work functions and duties may be assigned.
PARTICIPANT CARE (Approximately 60% of Time)
•Performs home visits to administer prescribed medications and treatments in accordance with nursing standards that supplement clinic services.
•Posts advanced directives in participants' homes and educates participants and families regarding plans of care.
•Instructs participants and family members regarding medications and treatment instructions in the home by CNA's, PCP's and homemakers.
•Performs home visits to supervise non-skilled services provided.
•Observes, records, and reports participant's condition/reaction to drugs and treatments to physicians.
•Informs Home Care Coordinator and team on home care issues of participants as they arise.
•Performs phlebotomy functions for participants and ensures that specimens are identified and labeled appropriately.

ADMINISTRATIVE (Approximately 40% of Time)
•Responds to and refers incoming telephone calls to appropriate parties.
•Maintains a log of all contacts with participants, either in the form of a phone log or a log of visits for Data Pace.
•Prepares/copies and faxes paperwork to contracted agencies that will provide home care services to participants.
•Maintains the personal supplies and materials needed for home visits, prepares a list of medical supplies needed and in stock.
•Attends meetings assigned by the Home Care Coordinator.

•Does not enter into any contract without approval which commits the organization to any obligation, or which transfers company assets to any outside interests, or which involve expenditures of a capital nature.
•Performs within position and personal limitations and provides information to employees, co-workers, business contacts, and others only as able and appropriate for position.

Requirements Qualifications

Education and Experience
•State of Colorado registration as a Licensed Practical Nurse required.
•Current CPR certification or the ability to obtain CPR certification required.
•A minimum of one year experience working with the frail or elderly required.
•Requires personal transportation, valid state issued driver's license, good driving record and auto insurance as required by law.

Computer Skills
•Must be computer proficient and possess experience with various computer systems and Microsoft Outlook.
•Must be able to quickly learn specific software and new applications.

Mathematical/Financial Skills
•Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
