DescriptionPosition Summary
Under the supervision of the VP of Home Care, the Denver Program Manager and the Area Manager, the Personal Care Worker (PCW) is responsible for providing and assisting participants with activities of daily living, restorative and supportive care necessary to the preservation of the home environment. Essential Functions and Work Duties The following essential functions of this position are performed personally, in cooperation with your supervisor, and/or in coordination with other staff. Additional work functions and duties may be assigned. PARTICIPANT CARE •Performs tasks as assigned by the Home Care Services Registered Nurse as instructed, ensures that instructions given are thoroughly understood. •Aids participants with the Instrumental Activities of Daily Living (IADL) that include; bathing, dressing, grooming, hair care, mouth care, eating, hand/foot nail care and toileting. •Assists participants with restorative and supportive care that include; ambulating, positioning and transferring. •Maintains order and cleanliness in participants' home by assisting participants with cleaning, changing bed linens, laundry, meal preparation, grocery shopping and other routine household chores as assigned. •Develops/collects shopping lists from participants, purchases and stores items in participant homes. Gathers appropriate input from the interdisciplinary team regarding the health concerns, personal wishes and status of the personal needs account. •Orders supplies from the Adult Day Health Center (ADHC) as needed for the performance of household chores. •Reports pertinent participant information regarding participants in a timely manner to appropriate team members and acts promptly in the case of emergency situations. •Sends all receipts for purchase of groceries and home supplies to the Accounting Department and follows appropriate accounting systems for purchasing items for participant care. •Documents services provided to participants in a timely and consistent manner utilizing the CareWatch and DAR systems. Completes flow sheets for medical records in a timely manner when CareWatch is down. •Maintains a safe environment for participants and self and displays awareness of, and reports, safety and/or reverse hazards. •Participates in participants' care planning through cooperation with multidisciplinary teams. Attends and successfully completes all mandatory trainings. •Does not communicate with any news media or volunteer business information to other agencies. Directs public relations issues to the appropriate person. •Does not enter into any contract without approval which commits the organization to any obligation, or which transfers company assets to any outside interests, or which involve expenditures of a capital nature. •Performs within position and personal limitations and provides information to employees, co-workers, business contacts, and others only as able and appropriate for position. Education and Experience •High School Diploma or GED required. •A minimum of one year working with the frail or elderly is required. •Requires personal transportation, valid state issued driver's license, good driving record and auto insurance as required by law. Computer Skills •Must be computer proficient and possess experience with Microsoft Word, Excel, and Outlook. •Must be able to quickly learn specific software and new applications. Mathematical/Financial Skills •Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. Language Skills •Ability to read, analyze and interpret regulations and other documents. •Strong interpersonal skills and ability to effectively and tactfully present information to, and communicate with, co-workers, employees, and others. •Possess exceptional English written and verbal communication skills, including accurate grammar and business correspondence knowledge. •Ability to read and write memos, reports, and correspondence that conform to prescribed style and format. Physical Requirements •Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination. •Requires standing, walking, pushing, bending, kneeling and reaching at arm's length and overhead in community for prolonged periods of time. •Must be able to transfer participants needing total assist. •Must be able to make home visits to participants who live in non-handicapped accessible dwellings. •Requires the ability to lift/carry up to 65 pounds and push or pull objects exceeding 150 pounds, using appropriate body mechanics. •Requires intermittent driving throughout the day, between participant's homes and the centers. Visual, Hearing and Communication Requirements •Requires corrected vision, including close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus. •Requires hearing to normal range and must be able to communicate effectively verbally and in writing with co-workers, vendors, participants, and others for work-related purposes. Materials, Tools Equipment, Work Aids •May require use of wheelchair, medical equipment, gait belts, sliding board, bedside commode, shower equipment, and other work aids. Environmental Conditions •May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, cleaning supplies, communicable diseases and any other conditions common in a home environment. •May be Subject to unpleasant odors and living conditions of participants. •May be subjected to adverse driving conditions including wet, icy, slushy and snow-packed roads.
