About This Position

Mercy Hospital Folsom, a member of Dignity Health, is looking to add a Director of Case Management to their hospital team. This position will oversee the Case Management and Medical Management Initiatives of Mercy Hospital of Folsom.

Requirements:
The position is accountable for coordinating the activities and functions of the Case Management department and for overseeing the departments day-to-day operations at MHF, with 24/7 accountability.

The incumbent will consult and collaborate with other managers, physicians, administration, and healthcare workers regarding care management issues identified through corporate or facility initiatives and current literature.

This position will manage the department in an efficient manner to ensure adequate coordination, discharge planning, and utilization review resulting in quality patient care, consistent with the Mission and Philosophy of Dignity Health.

Ensures that all actions taken and methods to produce results incorporate and promote the Dignity mission, Vision, Values, and MHF value-based service standards.Quality/Case Management experience and experience in a supervisory position, or a comparable combination of education and experience.

5 years BSN or related BS degree Business Administration or related health or management field or Masters degree.
Certification by professional organization.
CA RN License Required
