JOB SUMMARY:Medical Social Worker I is responsible for addressing psychosocial, financial and discharge planning needs of patients/ participants in the Map program. This includes, but is not limited to, Advance Directives issues, patient/family advocacy, medication assistance and coordination of volunteers for the Map program as well as other community resources.

QUALIFICATIONS:Current Iowa LBSW required. Case Management Certification required or willing to get within two years. 1 – 3 years professional social work experience preferred. Computer skills including Word, Excel, Cerner Power Chart. Ability to establish and maintain working relationships. Ability to work effectively as a team member and facilitate groups. Demonstrated ability to operate in a collaborative, shared leadership environment. Excellent communication skills including public speaking, presentation and writing skills. Ability to work under time pressure. Able to set and organize own work priorities and adapt to them as they change. High degree of initiative and judgment. Ability to work flexible hours. Ability to develop and maintain professional relationships with wide variety of individuals. Must provide proof of Basic Life Support-American Heart Association (BLS Healthcare Provider or HealthStream Heart Code) or American Red Cross (Professional Rescuer) via current card prior to employment. Maintains current certification in Basic Cardiac Life Support. Completes Mandatory Reporting requirement within 6 months of hire. Renews Mandatory Reporting requirement every 5 years.
