THIS POSITION WILL FLOAT BETWEEN MERCY NORTH IOWA ACUTE CARE SOCIAL SERVICES, MERCY HOME CARE, AND HOSPICE OF NORTH IOWA.

JOB SUMMARY: Medical Social Worker II is responsible for addressing psychosocial, financial and discharge planning needs of patients. This includes, but is not limited to, Advance Directives issues, patient/family advocacy, in-home visits and coordination of community resources. Provides services to Medical Social Worker I positions. Functions with a high level of independent judgment using skills and knowledge acquired through advanced education and experience to enhance patient care.

QUALIFICATIONS: Current Iowa LMSW or LISW required. Masters in social work degree required. Case Management Certification required or willing to get within two years. Two years professional social work experience preferred. Community or home health experience desirable. Computer skills including Word, Excel, Cerner Power Chart. Ability to establish and maintain working relationships. Ability to work effectively as a team member and facilitate groups. Demonstrated ability to operate in a collaborative, shared leadership environment. Excellent communication skills including public speaking, presentation and writing skills. Ability to work under time pressure. Able to set and organize own work priorities and adapt to them as they change. High degree of initiative and judgment. Ability to work flexible hours. Ability to develop and maintain professional relationships with wide variety of individuals. Must be able to function professionally in client’s residence where conditions may be less than desirable. Must provide proof of Basic Life Support-American Heart Association (BLS Healthcare Provider or HealthStream Heart Code) or American Red Cross (Professional Rescuer) via current card prior to employment. Maintains current certification in Basic Cardiac Life Support. Completes Mandatory Reporting requirement within 6 months of hire. Renews Mandatory Reporting requirement every 5 years.
