Job Description: **Please note that this is not a PeaceHealth position** TheVolunteers In Medicine Clinicis a not for profit organization that provides high quality medical and health services for workers who cannot afford health care or who cannot qualify for government assistance. We provide the facilities, organizational structure, and management, so that volunteering medical professionals can conveniently provide medical services in a welcoming and non-judgmental environment. The clinic emphasizes health education and prevention and networks with other health care agencies and institutions to which clients can be easily referred for additional care. We operate under the Circle of Caring which is love freely given. We believe that both the care givers and the recipients of care can be healed and nourished in this environment. As a collaborative team member, thisNurse Practitionersupports the philosophy of providing quality professional care for patient's medical, physical and psychosocial needs. In addition, this position provides diabetes self-management training to patients in the clinic and promotes behavior change in lifestyle and medication adherence to maximize health status of referred patients in an individual or group setting. PRINCIPAL DUTIES AND RESPONSIBILITIES: * Obtains patient's medical and psychosocial history. Performs physical and emotional assessments. Provides needed procedures and continually assesses patient's needs and conducts appropriate follow-up. * Provides health education for defined population. * Orders appropriate screening and diagnostic testing, pharmacological and durable goods, recognizing limited resources available at VIM. Appropriately recommends consultation/referrals with specialists in efficient manner. Follow up on all lab work, imaging studies and other diagnostic studies. * Performs minor procedures. * Counsels patients regarding general physical and emotional concerns, including health habits and disease history within the resources of the clinic. * Maintains a complete, accurate, and legible medical record. * Identifies situations that are beyond the nurse practitioner's scope of practice and refers these to the Medical Director. Demonstrates collaboration with other providers in dealing with clinical issues. * Promotes, establishes and maintains open and effective communications with all members of the team to solve problems, to identify mutual areas of concern, and to assess resources. Attends clinic meetings to coordinate care and presents clinic matters (reports patient condition and/or concerns) in a concise and complete manner. * Maintains confidentiality of patients, employees and volunteers. * This job may have additional responsibilities as assigned. Qualifications: KNOWLEDGE, SKILLS AND ABILITIES REQUIRED: * Current Oregon Nurse Practitioner with national prescriptive drug privileges with a DEA number. * Minimum two years experience as a Nurse Practitioner in a general medical field. * Excellent clinical assessment and treatment skills, with experience in dealing with women health issues (preferred). * Demonstrated ability to effectively teach in a group setting and utilize a variety of educational methods. Ability to lead, train volunteers and provide patient education. * Bilingual/bicultural preferred in working with Latino patients. * Excellent communication and interpersonal skills required. * Ability to work collaboratively with others, while maintaining flexibility to provide efficient work flow systems as well as develop camaraderie among team members. * Demonstrates a caring, respectful and compassionate attitude towards all people. * Takes responsibility for personal growth. * Takes initiative to meet the challenges within the environment. * Oriented toward solutions through collaboration. WORKING CONDITIONS: * Generally works in a normal office environment where there are minimal hazards and infrequent environmental changes. * In the unique volunteer environment of the clinic, there will exist a variety of situations that contain ambiguity, reinforced training, and the continuing need for mentoring and encouragement of co-workers and volunteer personnel. * The nature of the work may require frequent walking, standing, etc., as well as exposure to all areas of the Clinic. * Risk of exposure to bloodborne pathogens requires strict adherence to Bloodborne Pathogen Exposure Plan. REPORTING RELATIONSHIPS: Reports to the Medical Director.
