Immediate need for a BC/BE Family Practice physician to join a Hospice Care organization. The incoming Physician will be routinely scheduled to work M-F, 8A ? 5P Providers day will start by reviewing the overnight report and making rounds in the 16-bed care center to treat any acute patients. Then, based on the report and scheduled patient visits, the Physician and the (ARNP) will determine the needs and schedule for the day. ARNP has been with organization for over ten years and is extremely knowledgeable in hospice care. The Physician?s schedule will be a combination of seeing patients in the care center and also seeing patients in their place of residence (nursing homes, homes, and potentially hospital consults). They cover a census of approximately 145 patients enrolled in hospice care and the Physician and ARNP can expect to be in the care center and seeing patients in their place of residence as it makes sense based on acuity. The Physician will work in a team-oriented environment, which includes an ARNP, LPNs, CNAs, RN Case Managers, PRNs, Chaplains, Social workers and a host of Volunteers that care for the patients and their families. In addition to seeing patients, the Physician will attend weekly IDG meetings, Pharmacy and Therapeutic meetings once per month, and monthly Medical Meetings where they will discuss practice logistics and the on-call schedule. Excellent compensation!!!!
