Nurse Manager, Skilled Nursing and Palliative Care Job Listing at St. Francis Medical Center in Lynwood, CA

Develops, implements, coordinates and evaluates the delivery of nursing and multidisciplinary services in the SNF unit and for Palliative Care Services.  Ensures the delivery of safe, high quality nursing care and exemplary service to patients and families. 

This position requires the ability to assess, plan, implement and evaluate nursing practice and provide nursing management and leadership for a patient care unit which provides care/service to an acutely ill adult patient population that requires short-term, skilled nursing, patient care services in a manner that demonstrates an understanding of the functional and/or developmental age of the individual served.  In addition, the position provides managerial/administrative supervision within a service line model.  Provides support to patients and associates on all adult units for Palliative Care screening, intervention and care planning.
EDUCATION/TRAINING/EXPERIENCE
Bachelor's degree in Nursing required
Master's degree preferred
Three years of nursing experience in a long-term care environment required
Distinct-part SNF experience preferred
Min. one year experience in management of short term rehabilitation/skilled nursing unit
Strong knowledge of State and Federal/CMS guidelines and regulations pertinent to management of CMS reimbursement
Excellent written/verbal communication skills
Fiscal, staffing and operational skills in managing a nursing unit
Quality Improvement and nursing professional practice development knowledge
Meets the CMS, Joint Commission and Title 22 requirements for SNF Nurse Manager
