Under the supervision of the Patient Care Supervisor, provides nursing care in coordination with a multidisciplinary team. Uses the nursing process of assessment, goal setting, care planning, intervention, and evaluation to assist the patient to meet his/her highest level of functional independence and well being within a realistic rehabilitation potential. Provides supervision and caregiver instruction and community referral to enable the patient's ongoing dependent care needs to optimize and be accountable for patient safety in the home. Incumbent has access to Protected Health Information as needed to accomplish the duties outlined in this job description. This includes access to the entire medical record. Complies with and participates in Joint Commission Patient Safety Initiatives.
