Description
The Regional Care Coordination Executive is accountable for oversight of the planning and execution of regional care management activities including defining region-based priorities, overseeing implementation, monitoring key performance indicators, measuring results, allocating resources and building strategic partnerships to improve care coordination and transitions of care within the region, inclusive of all acute care facilities, long term care units/facilities, medical groups and partners e.g. IPAs, community partnerships. The incumbent is responsible for the development of systems and day-to-day operations of the region’s care coordination function. The incumbent is also responsible for leading system-wide and region-based best practice work within the region as it relates to care management healthcare resources, continuity of care and clinical outcomes of all case management and social services in both inpatient and outpatient settings including: inpatient palliative care, RAC response and management, referrals and authorizations, utilization review, discharge planning, social services, disease and population management programs. Additionally, the executive promotes solutions that accelerate the dissemination of successful best practices within the region, promoting personalized medicine and creating organizational alignment around care management. Deliverables result in measurable improvements in quality, service and efficiency across the organization for the benefit of patients, caregivers and customers.

The Regional Care Coordination Executive is a leader in the organization’s pursuit to transform healthcare to achieve the highest levels of quality, access and affordability. The incumbent plays a critical key role in strategic initiatives including but not limited to Total Care Accountability as well as our pursuit of new product offerings and partnerships e.g. healthcare reform-related opportunities.

This position reports to the Region Operations Lead—the Regional Chief Medical Officer or Chief Administrative Officer—but has a dual accountability to the system Vice President of Care Coordination
