The Medical Center also provides professional education and basic biomedical, clinical and behavioral research. Today, with its 1,254 licensed acute care beds and 25 residential beds, California Pacific Medical Center is one of the largest private, non-profit academic medical centers in California.
As a member of the Outcomes Management team, the Case Manager ensures appropriateness of admission and level of care; promotes effective utilization of clinical, ancillary and material resources; coordinates, plans, implements and evaluates the patient care and discharge plan for each patient based upon initial and ongoing assessments; collaborates with the mulitdisciplinary team to move the patient along the Care Continuum when appropriate; and collaborates with the multidisciplinary team to define and achieve optimal clinical and resource outcomes. The general duties and essential functions of the Case Manager role require: A. Collaboration with the multidisciplinary team in planning, coordinating, implementing and evaluating all aspects of Case Management including utilization management and discharge planning. B. Conducts initial concurrent review process for all selected admissions to initiate the tracing process, documenting findings on DRG worksheets, and identification of other key pathway or quality indicators as appropriate. C. Assists with monitoring of population based guidelines. Outcomes Management Services are provided 7 days per week including holidays. Important interfaces occur across all aspects of the Medical Center including Outcomes Management Team, Information Systems, Financial Management and all clinical departments. Education: A. Current Licensure as a Registered Nurse in the State of California required. B. Master's degree preferred C. BSN required. (Will waive education requirement for current incumbents in Case Management role or those with 5 years related experience)
