Job Title: Access Care Manager - 7p-7a Job ID: 126513 Medical Center, Houston, TX Location: Full/Part Time: Full-Time Regular/Temporary: Regular Shift and Salary Grade Shift: 7p-7a Salary Grade: 13 Position Summary Overview: Functions as a member of the interdisciplinary healthcare team in the provision of patient care with the underlying objectives of enhancing the quality of clinical and financial outcomes and patient satisfaction while managing the plan of care. Within the purview of financial management, pro-actively communicates with members of the healthcare team to ensure appropriate medical necessity for patients in beds; Serves as an effective liaison to ensure continuity and integration of services and accurate assignment of patient status. Communicates with payers and patient/family/healthcare team regarding payer information as necessary. Ensures that care moves through the episode of care in a timely and efficient manner. Responsibilities: High level screening, assessment, and planning to enhance and support Emergency Center patient flow. Ensures quality, cost-effective use of resources and facilitates access to care. Serves as a liaison between members of the interdisciplinary care team; coordinates appropriate and timely placement of internal and external transfers for all patients within the acute care setting. Effectively and efficiently oversees patient access to optimally assure clinical and financial outcomes. Demonstrates sound, effective, interpersonal, communication, and professionalism skills. Knowledge and Skills: Required for completely satisfactory performance in this job is knowledge of specific nursing processes, anatomy and physiology. Excellent verbal and written communication skills. Analytical decision making and judgment skills. Demonstrated ability to function as a clinical care team leader. Knowledgeable of all clinical resources available to patients both inpatient and outpatient. Data Entry and Word Processing Skills. Education, Experience and Licensure: The formal education and experience required for completely satisfactory performance in the job is: An Associate's degree or diploma in Nursing Current RN licensure by the State of Texas Nursing Board 3 years of nursing experience as a Registered Nurse in an acute care setting The following is preferred: BSN Pediatric and case/care management experience Case Manager certification About Texas Children's Texas Children's Hospital is an internationally recognized full-care pediatric hospital located in the Texas Medical Center in Houston.
