Job Title: Pharmacist (The Center) Job ID: 126660 Medical Center, Houston, TX Location: Full/Part Time: Full-Time Regular/Temporary: Regular Position Summary Overview: Promote pharmaceutical care by managing the medication process, identifying, preventing and resolving actual drug-related problems. Perform the duties of a registered pharmacist for patients ages 0 to adult. Responsibilities: Ensure appropriateness of medication doses in reference to the patient's diagnosis, weight/body surface area, allergies and drug interactions; enter medication orders and ensure orders are processed and patient receives correct medications; follow aseptic technique and infection control in the preparation of sterile products. Participate in the education of patients, care givers and the community by providing drug information; participate in the training of new employees, education of pharmacy students and the education of other healthcare professionals; annually publish an article in a professional journal/newsletter or make a presentation at a community or professional meeting. Support patient safety systems by completing intervention documentation; initiate and process medication incident and adverse drug reaction reports; perform and document patient profile review; complete pharmacist code response intervention; perform and document discharge prescription review; complete VI data collection form; enforce the non-formulary medication guidelines. Act as a liaison between the pharmacy, nurses and physicians. Engage in shift management by ensuring the pharmacy runs smoothly and by maintaining an adequate stock of medications and supplies; coordinate on-line activities, including technician workload, managerial duties and shift transition; re-establish priorities based on workload and patients' needs. Maintain investigational drug records. Knowledge and Skills: Medication knowledge base, knowledge of pediatrics and proper handling of hazardous substances required. Must be able to work under pressure and work closely with others continuously throughout the day. Education, License and Experience: Degree in pharmacy and Texas pharmacist license required. Department Marketing Statement The Center at Greenspoint, expected to open August 2013, is a patient and family-centered medical home developed as an innovative, comprehensive, and coordinated primary care practice exclusively for TCHP members (Children and Pregnant Women). Designed to improve access to high quality health care for the Medicaid and Children's Health Insurance Program (CHIP) populations. The Center will have full service dentistry, optometry, pharmacy, imaging, as well as, adult and pediatric behavioral health services. The Center will feature concierge level service optimizing the patient care experience, comprehensive coordinated care with full support services enveloping the care team (social workers, behavioral health, care coordinators, patient navigators, nutritionists, case managers, nurse triage, and intense appointment services), and Medicaid and CHIP re-certification and renewal assistance. The Center will be open for pediatrics 16 hours per day Monday thru Friday and 10 hours per day on weekends and holidays, and will be open for OB visits 12 hours per day Monday thru Friday and 6 hours per day on Saturdays and holidays.
